| 内容記述 | 日本オストミー協会A県支部に所属している120名のオストメイトを対象に、オストメイトの加齢に伴うセルフケアのニーズを明らかにすることを目的に、セルフケア状況、支援体制、および、セルフケア継続のニーズについて質問紙調査を行った。回収率は66．7％であった。対象者の平均年齢は71.5±1.2歳であった。オストメイトが困難に感じているセルフケアは、「装具の購入や支給の手続き」、「面板の穴の調整」と「面板の貼り方」であった。高齢オストメイトは後者2項目に加え、「排出口の開閉」も困難に感じていた。医療者から受けていた支援は、「装具の購入や支給の手続き」、「装具交換時の排泄物の流出防止」と「皮膚の洗浄」であった。家族から受けていた支援は、「袋から排泄物を捨てるタイミング」と「排出口の開閉」であった。オストメイトは将来、在宅と施設の両方で支援を受けたいと望んでいた。オストメイトの80％以上に家族支援者があり、その66％は配偶者であった。家族支援を受けているオストメイトは受けていないオストメイトより健康なうちに家族にストーマケアを教えたいという希望が高かった。また、灌注排便法と自然排便法を併用しているオストメイトは自然排便法のみを実施しているオストメイトよりセルフケアを困難に感じていた。 以上の結果から、オストメイトには困難に感じているセルフケアの情報提供や技術指導、在宅や施設において受けられる介護サービスに関する情報提供、健康なうちに家族支援者にストーマケアを教えることの啓蒙などの支援の必要性が示唆された。また、灌注排便法を実施しているオストメイトには身体機能が健全な間に自然排便法の再修得を支援する必要性が示唆された。 |

| 内容記述 | Aiming to demonstrate the needs for self-care of ageing ostomates, we conducted a questionnaire survey on 120 ostomates affiliated to the “A” prefecture branch of the Japan Ostomy Association regarding their self-care, the support system, and the needs for continuing self-care. With the response rate of 66.7％, the survey yielded the following results. Ostomates faced some difficulty in their self-care, such ａs “procedures for purchasing stoma appliances and obtaining the benefit” “adjusting the hole on the face plate” and “applying the face plate”. The seif-care assisted bｙ the medical staff consisted of “procedures for purchasing stoma appliances and obtaining the benefit”, “preventive measure against soiling the skin when applying the face plate”, and “skin cleansing”. The family members assisted ostomates with such self-care as “timing to empty the pouch” and “opening and closing the pouch”. The respondents expressed their hope for receiving the support both at home and institutions. Over 80％ ostomates had family supporters, 66％ of whom were spouses. In addition, ostomates who relied on irrigation needed support to reacquire the natural bowel evacuation while their body functions were maintained. These survey results exhibited that, in addition to the general information provision and technical guidance, ostomates needed the information regarding the nursing care services available at home and institutions, and the education for the family supporters on stoma care. They also indicated the necessity to develop the screening method of ostomates facing difficulties with self-care. |
